DATE, enter the date using this format: 01/01/10
LAST NAME, enter the driver's last name & first initial.
LOCATION, enter the street name, or intersecting streets.

Reports are generally uploaded within 3 - 5 business days. If, after five business days the report you are looking for is not located please call the Records and Information Section at

Monday-Friday 10:00 AM to 6:00 PM.
